a true and correct copy of 1) the August 15, 2003 Order Approving Amended
Disclosure Statement and Setting Plan Confirmation Deadlines, and Notice
Thereof, 2) Debtor's Amended Disclosure Statement, 3) the Debtor's Amended Plan
of Reorganization, 4) a ballot, and 5) a Declaration of Service to all
creditors on the creditors mailing matrix, a copy of which is attached hereto
as Exhibit A.
